Case History and Presentation
Amy, a 36-year-old woman, presents with worsening symptoms of AD during the past year. Despite using prescribed topical corticosteroids and emollients, she reports persistent redness, itching, and scaling. The itching has become so severe that it disrupts her sleep. She also mentions that the treatments seem to work initially, but their effectiveness diminishes over time. Amy is frustrated and expresses dissatisfaction with her current therapy, stating, “I feel like I’m constantly chasing my symptoms, and nothing really works.”

Amy was diagnosed with moderate AD at the age of 5, now categorized as severe. On the physical exam, she has widespread erythematous plaques with excoriations, lichenification, and evidence of postinflammatory hyperpigmentation. Laboratory tests show an elevated immunoglobulin E level and eosinophilia.

What is the most appropriate next step in the management of Amy’s AD?
