Released: October 30, 2026

About This Tool and Disclaimer

Decera Clinical Education is grateful to John Mascarenhas, MD; Abdulraheem Yacoub, MD; Brady L. Stein, MD, MHS; Kristen Pettit, MD; and Raajit Rampal, MD, PhD, who provided expert guidance and support in developing this Interactive Decision Support Tool.

The tool was developed in September and October 2026.

Recommendations reflect individual faculty opinions for the scenarios presented and may differ from one another, guideline recommendations, or FDA-approved labeling.

The goal is to help healthcare professionals evaluate management options for adults with chronic-phase primary MF or post–polycythemia vera (PV)/post–essential thrombocythemia (ET) MF. The tool addresses selected common scenarios and does not replace individualized clinical assessment.

Depending on the pathway, the questions address blast percentage, prognostic risk, transplant candidacy and preferences, symptoms or splenomegaly, elevated blood counts, prior JAK inhibitor therapy, platelet count, anemia/transfusion status, the reason for a treatment change, and serum erythropoietin (EPO).

Other factors that may affect treatment include comorbidities, performance status, renal and hepatic function, bleeding and thrombotic risk, infection history, concurrent medications, prior treatment duration and dose, molecular/cytogenetic findings, treatment access, and patient goals.

Related learning resources: online modules by Abdulraheem Yacoub, MD, and Sara Leidy, PharmD, BCOP, and a patient toolkit for navigating MF.
