Optimizing Prevention and Management of Tumor Lysis Syndrome in Patients With Hematologic Malignancies: A Grand Rounds Workshop Series

These slides review the pathophysiology, clinical consequences, risk assessment, prevention, and management of tumor lysis syndrome (TLS) in patients with hematologic malignancies, with an emphasis on identifying patients at increased risk and applying evidence-based monitoring, supportive care, and uric acid–lowering strategies. They address practical management of TLS-associated electrolyte abnormalities and the roles of allopurinol, febuxostat, and rasburicase, including dosing, safety, and patient-selection considerations. The slides also examine evolving TLS risks associated with newer therapies, including venetoclax, CAR T-cell therapy, and bispecific antibodies, and treatment-specific mitigation strategies such as dose ramp-up, prophylaxis, laboratory monitoring, and inpatient vs outpatient initiation. In addition, they highlight considerations for developing institutional TLS protocols to standardize risk stratification, prophylaxis, treatment initiation, and multidisciplinary management across clinical settings.
