Patients receiving CAR T-cell therapies and bispecific antibodies may present to the emergency department with time-sensitive adverse events, including cytokine release syndrome (CRS) and immune effector cell–associated neurotoxicity syndrome (ICANS), that can mimic sepsis, stroke, delirium, and other acute conditions. In this webinar, our expert faculty will explore practical strategies for recognizing and grading these toxicities, initiating first-hour stabilization and management, and coordinating timely escalation and handoff with oncology and other care teams.

Goal Statement
The goal of this activity is to improve learners’ ability to recognize, grade, stabilize, and coordinate care for patients presenting to the emergency department with suspected adverse events related to CAR T-cell therapy or bispecific antibody therapy.

Target Audience
This activity is intended for emergency medicine physicians, advanced practice providers, nurses, pharmacists, physician associates, and other acute-care healthcare professionals in community and academic settings who may evaluate patients presenting with adverse events related to CAR T-cell therapy or bispecific antibody treatment.

Learning Objectives
Upon completion of this activity, participants should be able to:

  • Identify early CRS, ICANS, infection or neutropenic fever, stroke, delirium, and metabolic encephalopathy in patients with recent CAR T-cell therapy or bispecific antibody exposure

  • Apply grading criteria to facilitate appropriate first-hour stabilization, monitoring, and escalation steps

  • Initiate evidence-based ED management of suspected CRS or ICANS, including parallel infectious and neurologic workup, supportive care, antimicrobial therapy when indicated, cytokine-directed therapy or corticosteroids when appropriate, and ICU or transfer decisions

  • Use treatment-history tools, wallet cards, EHR documentation, CAR T REMS update knowledge, product-specific bispecific REMS/monitoring requirements, and SBAR handoff to improve coordination among care teams
