The goal of this activity is to improve the knowledge and competence of learners in effort to optimize treatment for patients with Heart Failure.

Target Audience
This activity is intended for family medicine physicians, nurse practitioners, physician associates, and other primary care healthcare professionals involved in the care of patients with CKM diseases.

Learning Objectives
Upon completion of this event, participants should be able to:

  • Identify patients at elevated cardiovascular and kidney risk to support earlier, evidence-based risk stratification strategies in family medicine settings
  • Develop individualized, evidence-based treatment plans that incorporate cardio-renoprotective therapies to reduce CKM-related morbidity and mortality
  • Implement team-based and patient-centered care strategies to improve coordination, optimize adherence, and reduce progression of CKM-related complications
