Cases of Confounding Cholestasis: Navigating the Diagnostic and Therapeutic Challenges of PFICs

Progressive familial intrahepatic cholestasis (PFIC) and Alagille syndrome (ALGS) are rare, heritable cholestatic liver diseases that lead to elevated serum bile acid levels and a range of hepatic and systemic effects, including progressive hepatic injury and diminished quality of life. Debilitating cholestatic pruritus is a hallmark of these diseases. Although PFIC and ALGS are typically recognized in the pediatric setting, they can present across the entire lifespan. However, insufficient knowledge of these diseases, particularly among adult-focused healthcare professionals (HCPs), often leads to delayed or missed diagnosis, development of complications, and poor access to care among adults presenting with these diseases or transitioning from pediatric care. With greater access to genetic testing and growing evidence on the benefits of targeted therapies such as ileal bile acid transporter inhibitors, HCPs need to recognize patients who might benefit from testing for genetic causes of cholestasis and provide effective, evidence-based treatment. This interactive symposium, featuring small-group case-based discussions and immediate expert feedback, will equip hepatologists, gastroenterologists, and multidisciplinary healthcare teams with practical strategies for recognizing, diagnosing, and managing PFIC and ALGS in adult populations.

The goal of this activity is to improve learners’ knowledge and competence to identify, accurately diagnose, and comprehensively manage adult patients with PFIC and ALGS across the full disease spectrum. 

Target Audience
This activity is intended for hepatologists, gastroenterologists, nurses, nurse practitioners, pharmacists, and other healthcare professionals involved in the management of adult patients with or at risk of PFIC and ALGS.

Learning Objectives
Upon completion of this event, participants should be able to:

  • Assess for late-onset and adult-presenting genetic cholestatic disorders in patients with suspected mimics or established diagnoses that warrant reevaluation
  • Apply an evidence-informed diagnostic framework that incorporates systematic use of genetic testing to evaluate subtype-specific presentations in patients with suspected PFIC or ALGS across the lifespan
  • Devise individualized, evidence-informed treatment and symptom management strategies for adults with PFIC and Alagille syndrome
  • Implement a multidisciplinary care plan that integrates long-term disease monitoring, transition of care planning, and family-centered genetic counseling for patients with PFIC and Alagille syndrome
