Goal Statement
The goal of this activity is to improve the knowledge, skills, and performance of learners to promote better practices in preserving brain health and preventing cognitive decline.

Target Audience
This activity is intended for healthcare professionals practicing in the United States who care for patients at risk for cognitive decline, including primary care practitioners, nurse practitioners, physician associates, general neurologists, geriatricians, as well as a secondary audience of psychiatrists and OB/GYNs.

Learning Objectives
Upon completion of this activity, participants should be able to:

PCP

  • Apply evidence-informed strategies, including emerging blood-based biomarkers, to support early detection and assessment of cognitive impairment in aging patients
  • Initiate compassionate, patient-centered conversations about cognitive health with patients during routine wellness and chronic disease management visits, including consideration of age-related concerns
  • Design a patient-specific management plan to address modifiable risk factors for cognitive decline during patient visits, using a holistic, evidence-informed approach that considers age, lifestyle, medical, and psychosocial determinants

Specialist

  • Implement expert-recommended neurocognitive assessments and emerging diagnostic tools, including blood-based biomarkers, into clinical workflows to enhance early and accurate identification of cognitive decline in aging patients
  • Engage aging patients and their caregivers in personalized discussions about cognitive health, preservation strategies, and baseline assessments about modifiable risk factors for cognitive decline, fostering informed choices and collaborative care planning
  • Develop personalized, multidisciplinary care plans for patients at risk for or diagnosed with Alzheimer’s disease, targeting modifiable risk factors, based on evidence-based strategies to preserve cognitive function and delay progression
