APP-Led Management of HER2-, TROP2-, and B7-H3–Directed ADC Adverse Events in Solid Tumors: Practical Strategies for the Clinic

Antibody–drug conjugates are increasingly used across breast cancer, lung cancer, and other solid tumors, creating new safety challenges for oncology APPs who often assess symptoms, triage between visits, and help determine whether therapy should proceed, be held, or escalated. This activity uses practical, case-based discussion to examine HER2-, TROP2-, and emerging B7-H3–directed ADC adverse event patterns, CTCAE grading, ILD/pneumonitis evaluation, oral and ocular supportive care, neutropenia and diarrhea management, and APP-led workflows for patient counseling, referral, and multidisciplinary communication.

Goal Statement
This activity aims to improve learners’ competence and confidence in anticipating, identifying, grading, triaging, and managing adverse events associated with HER2-, TROP2-, and emerging B7-H3–directed ADC therapies in patients with breast cancer, lung cancer, and other solid tumors.

Target Audience
This educational activity is intended for advanced practice providers (APPs) in oncology, including nurse practitioners, physician associates, clinical nurse specialists, and other HCPs who assess, educate, triage, and manage patients with solid tumors receiving ADC therapy.

Learning Objectives
Upon completion of this activity, participants should be able to:

  • Differentiate toxicity patterns associated with HER2-, TROP2-, and emerging B7-H3–directed ADCs by integrating target, payload, linker, dosing schedule, patient comorbidities, and regimen context

  • Apply CTCAE grading and agent-specific guidance to ADC-related ILD/pneumonitis, stomatitis/oral mucositis, ocular toxicity, diarrhea, neutropenia, thrombocytopenia, nausea/vomiting, fatigue, and infusion-related reactions

  • Implement APP-led monitoring and triage workflows for early toxicity detection, dose interruption, initiation of supportive care, specialty referral, rechallenge consideration, and discontinuation when clinically indicated

  • Develop patient-centered counseling and interprofessional communication plans to improve timely reporting of ADC red-flag symptoms and coordination among multidisplinary care teams
