TROP2-Directed ADCs in NSCLC: Current Evidence, Emerging Therapies, and Future Treatment Strategies

Podcast

In this podcast, experts discuss the evolving role of TROP2-directed antibody–drug conjugates (ADCs) in NSCLC, highlighting current clinical applications, emerging efficacy and safety data, and future treatment strategies. The discussion reviews evidence supporting the use of datopotamab deruxtecan (Dato-DXd) in previously treated EGFR-mutated NSCLC and considerations for treatment selection and sequencing. Emerging data for investigational TROP2 ADCs, including sacituzumab tirumotecan, and findings from ongoing trials evaluating ADC combinations with immunotherapy, chemotherapy, and EGFR-targeted therapies are explored. The experts also address the potential role of the TROP2 QCS-NMR biomarker in identifying patients most likely to benefit from treatment, challenges in interpreting clinical trial outcomes, and practical strategies for preventing and managing adverse events, including stomatitis, ocular toxicity, and interstitial lung disease/pneumonitis. These considerations provide context for integrating TROP2 ADCs into clinical practice as evidence continues to evolve.

Target Audience

This activity is intended for medical oncologists, pulmonologists, and other healthcare professionals involved in the care of patients with advanced/metastatic NSCLC.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Evaluate the current and evolving role of TROP2-directed ADCs in advanced/metastatic NSCLC, including current evidence-based use of Dato-DXd and emerging investigational strategies across frontline, biomarker-enriched, and combination settings

  • Apply established biomarkers and patient-specific clinical factors to guide selection, sequencing, and clinical trial referral for TROP2-directed ADCs and ADC-based strategies in advanced/metastatic NSCLC

  • Appraise emerging global clinical trial evidence for TROP2-directed ADCs, including Dato-DXd, sac-TMT/MK-2870, ADC–immunotherapy combinations, ADC–EGFR TKI combinations, and biomarker-driven approaches

  • Implement multidisciplinary strategies to prevent, monitor, and manage TROP2 ADC–associated adverse events, including stomatitis/oral mucositis, ocular toxicity, nausea, myelosuppression, and ILD/pneumonitis
