Optimizing TROP-2 ADC Integration in NSCLC: Evolving Evidence, Safety Management, and Sequencing in a Changing Treatment Landscape

Slideset

Downloadable slides on the evolving role of TROP-2–directed antibody–drug conjugates (ADCs) in non-small-cell lung cancer (NSCLC), including recent clinical trial evidence, biomarker development, treatment sequencing, and adverse event management. The slides review differences in the mechanisms of action and safety profiles of datopotamab deruxtecan, sacituzumab govitecan, and sacituzumab tirumotecan, along with efficacy and safety findings from pivotal and emerging trials, including TROPION-Lung01/02/05, EVOKE-01/02, and OptiTROP-Lung01, 03, 04, and 05. Data are examined across patient subgroups defined by histology, EGFR mutation status, PD-L1 expression, and prior treatment, with consideration of approved indications, investigational combinations, and emerging TROP-2 biomarker approaches. The slides also address practical strategies for preventing, monitoring, and managing ADC-associated toxicities, including stomatitis, ocular adverse events, neutropenia, diarrhea, and interstitial lung disease/pneumonitis, providing clinical context for individualized treatment selection, sequencing, and supportive care in advanced NSCLC.

Target Audience

This activity is intended for oncologists and other healthcare professionals who care for patients with non-small-cell lung cancer.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Differentiate TROP-2 ADC platforms in NSCLC based on antibody target, linker, payload, drug-to-antibody ratio, dosing schedule, evidence maturity, and expected toxicity profile

  • Assess the current limitations of TROP-2 biomarker testing and the potential future role of exploratory quantitative and computational pathology approaches in individualized treatment planning

  • Interpret recent efficacy and safety data from pivotal and emerging monotherapy and combination trials of TROP-2 ADCs, including ESMO 2026 updates, with attention to histology, genomic driver status, PD-L1 expression, prior therapy, and regulatory status

  • Develop evidence-aligned sequencing strategies that distinguish approved use, guideline-supported options, and clinical-trial opportunities for patients with advanced NSCLC after immunotherapy, chemotherapy, and/or targeted therapy

  • Apply proactive, platform-specific monitoring and multidisciplinary management strategies for TROP-2 ADC–associated adverse events, including neutropenia, diarrhea, stomatitis, ocular toxicity, and ILD/pneumonitis
