Patient-Centered Management of Smoldering and Newly Diagnosed Multiple Myeloma

Clinical Thought

The care of patients with high-risk smoldering multiple myeloma (MM) or newly diagnosed MM requires an individualized, risk-adapted approach at every stage. This commentary covers evidence for subcutaneous daratumumab monotherapy from the AQUILA trial for high-risk MM, and the utility of quadruplets for patients with newly diagnosed MM based on the results from the PERSEUS, CASSIOPEIA, GMMG-HD7, and CEPHEUS trials, as well as support for intensified maintenance based on the AURIGA trial results.

Target Audience

This educational activity is intended for hematologists, oncologists, advanced practice providers, nurses, pharmacists, and other healthcare professionals involved in the treatment of patients with multiple myeloma.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Apply risk stratification models, diagnostic criteria, and guideline recommendations to identify appropriate patients for early intervention in high-risk SMM

  • Integrate practical treatment delivery considerations, including intravenous vs subcutaneous antibody administration, supportive care requirements, and monitoring protocols, to align therapy selection with patient needs

  • Recall emerging data on novel agents and combination strategies under investigation and appropriately identify patients who may benefit from referral for clinical trial participation

  • Individualize frontline treatment strategies for patients with NDMM by integrating emerging clinical evidence, guidelines, cytogenetic risk, comorbidities, transplant eligibility, functional status, and patient preferences to optimize short-term and long-term outcomes
