A 58-yr-old woman with no previous medical history is referred to hematology due to elevated mean corpuscular volume and leukopenia. Her laboratory results show: WBC 4.3 x 10⁹/L; hemoglobin 14.9 g/dL; platelets 252 x 10⁹/L; serum protein 7.5 g/dL; albumin 4.8 g/dL; creatinine 0.9 g/dL; IgG 1436 mg/dL; IgA 42 mg/dL; IgM 49 mg/dL; M-spike 0.9 g/dL; free light chain ratio 41.15. Serum immunofixation revealed IgG kappa. Workup using 24-hr urine showed 90 mg protein, no M-spike, and UIFE was positive for kappa light chains. Bone marrow biopsy showed: 40% kappa-restricted plasma cells. FISH analysis showed 1q+, del(13q). PET CT and MRI scans showed no bone lesions.
