In this text-based module, an expert reviews the progressive course of myelofibrosis and key considerations for individualized management, including treatment goals, assessment of spleen volume and symptom burden, use and monitoring of JAK inhibitors, and the role of hematopoietic stem cell transplantation. The module also addresses treatment-related toxicities, multidisciplinary care, and the importance of aligning therapy with disease characteristics, patient priorities, and evolving clinical needs, while highlighting clinical trial participation as an important avenue for expanding future treatment options.

A.S. is a 62-year-old woman with a recent diagnosis of primary MF. She presented with splenomegaly, drenching night sweats, and weight loss. Labs at presentation showed a white blood cell count of 14,000/µL with 3% circulating blasts, hemoglobin of 8.5 g/dL, and platelets of 95 x 109/L. She was not eligible for bone marrow transplantation because of lack of caregiver support. She received momelotinib 200 mg/day for 28 weeks. She developed mild neuropathy but chose to continue therapy. The frequency and severity of night sweats have increased and are interrupting her sleep. There was no change in spleen size or hematologic parameters. She is interested in a second-line therapy with an alternative JAK inhibitor. She has a history of recurrent shingles activation and frequent squamous cell skin cancers.

Which second-line approach best reflects shared decision-making for this patient?
