Target Audience

This activity is intended for heart failure nurses, nurse practitioners (NPs), physician associates (PAs), and other HCPs who care for patients with HF, including those with CKM conditions.

Program Learning Goal

The goal of this educational activity is to increase learners’ understanding of CKM diagnosis, management, and treatment, including the use of GDMT. 

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Integrate heart failure pathophysiology across the ejection fraction spectrum with contemporary diagnostic tools to improve diagnostic accuracy, enable earlier identification, and guide risk stratification

  • Evaluate the efficacy, safety considerations, and practical implementation steps for early and sustained use of GDMT in eligible HF patients, with attention to nursing assessment, patient counseling, CKM comorbidities, and interdisciplinary communication

  • Integrate guideline-directed medical therapy for patients with heart failure through nurse-led assessment, initiation support, protocol-driven titration, and ongoing monitoring of evidence-based treatments in alignment with current guidelines, while coordinating care to manage CKM comorbidities and treatment-related adverse effects
