Embracing Choice: The Role of Oral Regimens as a Bridge to Long-Acting ART

Clinical Thought

Historically, initial antiretroviral therapy for people living with HIV has prioritized rapid and durable virologic suppression with effective oral regimens. Although viral suppression remains the primary goal, long-term success also depends on treatment satisfaction, lifestyle considerations, and sustained engagement in care. Here, I discuss how the VOLITION study represents a contemporary HIV treatment journey in which initial oral ART naturally evolves into a choice regarding long-term treatment for maintenance of viral suppression. These findings support a patient-centered approach to HIV care, where achieving viral suppression is the foundation for subsequent treatment options that reflect individual needs, preferences, and lifestyle considerations.

Upon completion of this activity, participants should be able to:

  • Integrate the clinical evidence, including results from VOGUE and other key trials and real-world studies, to make informed, evidence-based decisions about the comparative merits of 2DR and 3DR for ART initiation

  • Apply evidence-based clinical frameworks to select between 2DR and 3DR across a range of commonly encountered first-line ART scenarios, and communicate the rationale for 2DR confidently in shared decision-making conversations with people living with HIV
