Beyond cBTKi Progression in R/R MCL: Mechanism-Based Sequencing and Community Implementation

Slideset

The treatment landscape of R/R mantle cell lymphoma continues to evolve as the use of covalent BTK inhibitors further expands into the first-line setting. These slides contain the latest data, guidelines, and expert recommendations for managing R/R disease post covalent BTK inhibitor. Highlights include the similarities and differences between small molecules pirtobrutinib and sonrotoclax including patient selection, step-up dosing requirements, TLS monitoring, efficacy post covalent BTKi, and implementing treatment in community practices. They also include the efficacy, safety, and best practices of CAR T-cell therapy and bispecific antibodies.

Target Audience

This activity is intended for hematologist/oncologists, nurse practitioners (NPs), physician associates/assistants (PAs), nurses, pharmacists, and other interprofessional team members involved in the care of patients with R/R MCL.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Evaluate patient-, disease-, and prior-treatment factors that inform post-cBTKi therapy selection in R/R MCL, including BTKi progression vs intolerance, relapse tempo, tumor burden, TP53 mutation, blastoid/pleomorphic morphology, Ki-67, performance status, comorbidities, access, and patient goals

  • Compare BTK- and BCL2-directed small-molecule strategies and contextualize CAR T-cell and bispecific antibody options using mechanism, efficacy, safety, treatment duration, monitoring needs, and implementation feasibility

  • Apply best practices for therapy-specific safety planning and supportive care, with emphasis on TLS risk assessment and monitoring for BCL2 inhibitor-based therapy, infection/cytopenia mitigation, drug interactions, referral readiness, and patient/caregiver counseling

  • Implement interprofessional, community-relevant care pathways that improve referral timing, shared decision-making, patient/caregiver education, monitoring adherence, and reassessment for patients with R/R MCL after cBTKi exposure
