The HF Huddle: Winning Strategies for Early Detection and Evidence-Based Treatment in Heart Failure

Slideset

It is essential that healthcare professionals can identify, phenotype, and risk-stratify heart failure early across the ejection fraction spectrum to inform treatment selection and improve outcomes. Use these slides to translate the current evidence into practical strategies for identifying and diagnosing heart failure, optimizing guideline-directed medical therapy, managing cardiometabolic comorbidities, and coordinating multidisciplinary care.

Target Audience

This program is intended for healthcare professionals involved in the diagnosis and management of heart failure (including patients with metabolic or kidney comorbidities), including cardiologists, including heart failure specialists, and cardiology-focused advanced practice providers (NPs/PAs) who frequently care for patients with heart failure.

These HCPs typically prefer practical, case-based education that supports clinical decision-making and translation of guideline recommendations into real-world practice. The proposed program is designed to address independently identified clinical practice gaps related to understanding heart failure pathophysiology across the ejection fraction spectrum, improving use of contemporary diagnostic strategies for earlier identification and phenotyping of heart failure, and optimizing implementation of guideline-directed medical therapy while managing metabolic and kidney comorbidities.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Apply current understanding of heart failure pathophysiology across the ejection fraction spectrum, including neurohormonal and cardiometabolic mechanisms, to inform clinical evaluation and risk stratification

  • Integrate contemporary diagnostic strategies, including biomarkers, advanced imaging, and validated clinical scoring systems, to identify heart failure earlier in disease progression and support accurate phenotyping

  • Implement guideline-directed medical therapy (GDMT) for patients with heart failure by strategically initiating, titrating, and sequencing evidence-based treatments in alignment with current guideline recommendations, while addressing cardiometabolic comorbidities and managing treatment-related adverse effects
